Existing drugs may help more breast cancer patients -Breast Cancer ...

More patients can benefit from highly effective breast cancer drugs that are already available, according to DNA sequencing studies by researchers at Washington University School of Medicine in St. Louis and other institutions.

The investigators found that some women with the HER2 negative subtype may benefit from anti-HER2 drugs even though standard tests don?t indicate they are candidates for the drugs.

?These patients are going to be missed by our routine testing for HER2 positive breast cancer,? says Ron Bose, MD, PhD, assistant professor of medicine. ?Currently they?re not going to receive a HER2 targeted drug because we don?t have a way to identify them. And we predict they are going to have a more aggressive form of breast cancer.?

Bose, who treats patients at Washington University?s Siteman Cancer Center at Barnes-Jewish Hospital, will present the data Dec. 7 at the San Antonio Breast Cancer Symposium.

Today, a type of breast cancer known as HER2 positive is treated with drugs that inhibit the function of the HER2 protein. To be classified as HER2 positive, a patient must have more than the normal two copies of the HER2 gene. Too much HER2 drives tumor growth and some HER2 positive patients may have as many as 20 copies of the gene. Doctors test for this gene ?amplification? in every patient diagnosed with breast cancer. It must be present for a woman to receive anti-HER2 drugs.

But instead of multiple copies of the gene churning out too much HER2, some patients deemed HER2 negative based on standard testing may have mistakes in just a few ?letters? of the DNA in their two gene copies that result in excess activity of the protein. Bose and his colleagues estimate that these undetected HER2 mutations ? rather than the HER2 amplification - may be driving tumor growth in 1.5 to 2 percent of all breast cancer patients. With about 230,000 new cases of breast cancer diagnosed in the United States each year, even this modest percentage translates into more than 4,000 patients per year.

The study, led by senior author Matthew J. Ellis, MD, PhD, of Washington University, and published online Dec. 7 in the journal Cancer Discovery, analyzed data from eight DNA sequencing studies, which together included about 1,500 patients. Two of the sequencing studies were conducted by The Genome Institute at Washington University, in collaboration with study co-author Elaine R. Mardis, PhD, co-director of the genome institute.

Of the 1,500 patients, 25 were found to have HER2 mutations without gene amplification. Not all mutations appeared to have the same effect, however. After analyzing 13 of the mutations, seven were found to drive cancer growth. In the laboratory analysis, most of these mutations responded well to the anti-HER2 drugs lapatinib and trastuzumab, both approved by the U.S. Food and Drug Administration. Although two of the mutations were resistant to lapatinib in lab tests, they responded well to neratinib, a newer anti-HER2 drug that is currently in phase II clinical trials.

Bose also cautions that some mutations were found to be ?silent,? meaning they did not drive the tumor?s growth and therefore would likely not respond to anti-HER2 drugs.

The study?s findings have led directly to the launching of a phase II clinical trial to test whether patients with HER2 mutations (but not the amplification) will benefit from anti-HER2 drugs. The trial will include patients with stage IV breast cancer classified as HER2 negative. Their HER2 genes will be sequenced to look for mutations. If mutations are present, they will be treated with neratinib in addition to the standard treatment they would otherwise receive.

At Washington University, the trial will be led by Cynthia X. Ma, MD, PhD, associate professor of medicine. The other centers participating in the study are the Dana-Farber Cancer Institute, Memorial Sloan-Kettering Cancer Center and the University of North Carolina, Chapel Hill.

Bose points to this study as an example of the potential value in sequencing the DNA of cancer patients, even when limited to a single gene of interest such as HER2.

?If we can identify mutations that we can act on, that information will help us better guide treatment,? Bose says. ?In this case, we don?t even have to develop new drugs against HER2 mutations. It?s just a matter of finding the patients.?

In the age of texting, it's easy to forget that the?cell phone was initially meant for making calls. That's still true, but most phones do a whole lot more than that nowadays. Not everyone wants or needs a smartphone, though, and it's getting harder and harder to find a quality alternative. Luckily, the free Samsung M400 for Sprint fits the bill. It's a super-simple flip phone with a large number pad and good call quality. While it's pretty much devoid of any multimedia features, it's a good choice for anyone who needs a phone primarily for talking.

Design and Call Quality
The Samsung M400 measures 3.84 by 1.99 by 0.72 inches (HWD) and weighs 3.65 ounces. It's made entirely of dark gray and black plastic, with a lightly textured panel on the back and a faux-metallic finish on the front. There's a Camera button on the right, as well as buttons for Volume and Voice Control on the left. The 1.07-inch external display shows the time, date, reception, and battery life at a glance, and will display the name or number for incoming calls.

Flip the phone open to access the M400's excellent number pad. The backlit keys are large, well-spaced, and springy. You won't have any trouble dialing here. The control pad above is equally well crafted. There's a directional pad with a Select button in the middle, flanked by buttons to start and end a call and activate the speakerphone. You also get a button that automatically dials 911 (you're asked if you actually want to dial it after your press it), as well as a programmable In Case of Emergency button. You can add up to three contacts you'd want to notify in the event of an emergency.

The internal 2.4-inch display features 320-by-240-pixel resolution. Text and images look sharp enough, but it's highly reflective, and narrow viewing angles make it tough to see if you aren't looking at it dead-on.

The M400 is a 3G device with no Wi-Fi. Reception is good, and call quality is solid across the board. Voices are clear in the earpiece, and volume goes extremely loud, but the louder you go, voices start to crackle and sound a bit fuzzy. Calls made with the phone sound very clear and easy to hear, with good noise cancellation. Calls were fine through a?Jawbone Era?Bluetooth headset, and the Nuance-powered voice control app worked very well. You can use it to make calls or send text messages, or even to open the phone's Web browser. The speakerphone sounds okay, and gets loud enough to use outside. Battery life was on the low side, at 4 hours and 56 minutes of talk time.

Apps, Multimedia, and Conclusions
The M400's UI is simple to navigate and responds quickly to key presses. The home screen displays all of the same information as the external display, and gives you quick access to your contacts and text messages. The main menu, meanwhile, features nine different shortcuts, including your camera, messages, Web, and app store. Don't get too excited about apps, though?there's barely anything in the store, and you only get 9.61MB of free internal storage, with no microSD card slot for expansion.

There are no email or IM clients, though you can still access email through the Access NetFront 4.2 browser. The browser delivers decent WAP pages, but it's really only good for occasional use. Instead of IMs, you're better off sending text messages. The M400 displays messages individually, or threaded, in the style of a conversation. You'll need to type by using the number keys, which is always a pain, but at least they're big and easy to press.

Multimedia features are nearly nonexistent. There's no music player, video player, or camcorder. You do get a 1.3-megapixel camera on the front of the phone. It lacks an LED flash and auto-focus, though it comes with some fun features like continuous shooting, mosaic, and panorama. Unfortunately, test photos look soft and hazy, almost like someone smeared Vaseline over the lens.?[Really? Ew.?Ed]?And since there's no memory card slot, the only way to get photos off the phone is to send them over the air.

So while the Samsung M400 doesn't have much in the way of extras, it isn't trying to be anything other than what it is: a cheap, simple voice phone. The only other flip phones on Sprint are the rugged Kyocera DuraCore ?and Kyocera DuraXT, both of which are far too big and bulky for casual users. The Samsung Array?gets you a keyboard and some extra media features, but has painfully slow 2G speeds for Web browsing, and isn't quite as simple to use as the M400.

Brain pacemakers show promise in Alzheimer's trials, might open new treatment possibilities

Deep-brain electrical stimulation has been around for awhile -- rousing folks from comas and helping researchers find new ways to treat OCD, depression and maybe even obesity. Now doctors are testing the so-called "brain pacemakers," on Alzheimer's patients and the results are promising, but not conclusive. After a year of treatment, six patients showed higher glucose consumption in PET scans of the learning and memory areas of the brain -- a change that is associated with increased neuron activity. Glucose metabolism typically decrease as a result of the disease. Researchers are seeking more patients for the trial, and are optimistic that the technique could one day be used to stave off some of Alzheimer's debilitating symptoms.

Factbox: What do the Doha talks mean for the carbon market?

(Reuters) - U.N. climate talks in Doha, Qatar, were unlikely to have any impact on depressed carbon markets, analysts said.

Extended debate gave the Kyoto Protocol, the world's only global pact on curbing climate change, a fragile lifeline.

But it did nothing to raise ambition on cutting emissions, which could have helped to reduce a surplus of offsets and emissions allowances that have crushed markets.

After years of haggling, the European Union came up with an internal agreement on how to deal with its surplus of international allowances, known as Assigned Amount Units (AAUs).

The EU text was agreed with minor changes in the final package of agreements known as the Doha Climate Gateway.

Under the EU deal, allowances would be carried over into a second Kyoto commitment period, but buying of them would be limited to 2 percent of the purchaser's national allocation.

There was no decision on cancellation after 2020 of surplus permits -- as demanded by developing nations.

But the handful of nations likely to purchase the allowances -- Australia, the European Union, Japan, Liechtenstein, Norway and Switzerland -- made declarations saying they would not buy them, ensuring they have limited market value.

CLEAN DEVELOPMENT MECHANISM

The $215-billion offset market allows governments and companies to invest in emission reduction projects in return for tradeable credits (Certified Emission Reductions), which they can use to meet international climate targets.

But the Clean Development Mechanism has suffered from the failure of the talks to deliver deep emissions cuts.

Prices have collapsed from above 22 euros ($28.44) in 2008, just before the failure of U.N. talks in Copenhagen to get nations to agree to binding emission caps, to about 50 cents.

A U.N.-commissioned report in September said that unless nations took on deeper cuts or a reserve bank was set up to buy surplus credits, the scheme could collapse.

Doha talks reached no agreement on a solution.

"As with Durban (climate talks) last year, the outcome will not surprise the market and so is unlikely to affect the price of EU allowances or CDM credits," said Jonathan Grant, director of sustainability and climate change at PwC.

Australian radio chair: Station reviewing policy

LONDON (AP) ? British police have contacted Australian authorities about a possible investigation into a radio station's hoax call to a U.K. hospital about the Duchess of Cambridge, they said Sunday, as the company that owns the station promised it is reviewing its broadcast practices.

The prank took a dark twist Friday with the death of nurse Jacintha Saldanha, a 46-year-old mother of two, three days after she unwittingly accepted the call about the health of Prince William's pregnant wife, Kate. The death has sparked an angry backlash from those who argue the Australian DJs who carried out the hoax should be held responsible.

The board of Southern Cross Austereo had an emergency meeting Sunday to discuss a harsh letter from the hospital that fell for the call. King Edward VII's Hospital, where the former Kate Middleton was being treated for acute morning sickness this week, condemned the "truly appalling" hoax and called the consequences "tragic beyond words."

Max Moore-Wilton, the chairman of 2DayFM owner Southern Cross Austereo, said in a letter to the hospital's chairman Sunday that the company will cooperate with any investigation.

"It is too early to know the full details leading to this tragic event and we are anxious to review the results of any investigation that may be made available to us or made public," he wrote. "I can assure you we are taking immediate action and reviewing the broadcast and processes involved."

"As we have said in our own statements on the matter, the outcome was unforeseeable and very regrettable," he wrote.

Australian police Sunday confirmed they had been contacted by London police and said they would cooperate.

The radio station callers impersonated Queen Elizabeth II and Prince Charles and received confidential details about the former Kate Middleton's medical information. The call was recorded and broadcast.

Police have not yet determined Saldanha's cause of death, but people from London to Sydney have been making the assumption she died because of stress from the call.

Both DJs involved apologized for the prank before Saldanha's death. Their show has been suspended indefinitely and their Twitter accounts have been taken down after they were bombarded by thousands of abusive comments.

The station has a history of controversy, including airing a segment in which a 14-year-old girl revealed that she had been raped. It also ran a series of "Heartless Hotline" shows in which disadvantage people were offered a prize that could be taken away from them by listeners.

The Australian Communications and Media Authority, which regulates radio broadcasting, says it received complaints from around the world and is considering whether it should launch an investigation

Separately, Prince William on Sunday pulled out of attending the British Military Tournament, billed as "the largest display of military theatre in the world", citing Kate's illness.

Officials from St. James's Palace have said the duchess is not yet 12 weeks pregnant. The child would be the first for her and William.
